Recently, at least 18 provinces across China are actively advancing the provincial-level pooling of medical insurance funds—a significant step toward a more integrated and equitable healthcare security system. Provincial-level pooling means unifying medical insurance policies, fund management, and benefit standards within a province, replacing the previous fragmented model based on cities or even counties. This reform enhances the risk-resilience of insurance funds, narrows regional disparities in coverage, and supports labor mobility and balanced regional development.Historically, differences in local economic conditions led to wide variations in insurance contributions and reimbursement rates, creating barriers for patients seeking care across regions—such as complicated claims processes and unequal benefits. With provincial pooling, a ‘one-province, one-standard’ approach will streamline cross-regional reimbursement and improve fairness and sustainability of the system. Centralized management also boosts fund efficiency and mitigates risks of local fund insolvency due to aging populations or public health emergencies.Challenges remain, however, including fiscal pressures on local governments, integration of information systems, and coordination of vested interests. Success will require strong collaboration between central and local authorities and robust supporting mechanisms. Overall, provincial-level medical insurance pooling is a crucial move in deepening healthcare reform and building a high-quality, universal insurance system—key to achieving common prosperity.
